Support Coordination

Support Coordinators assist you to understand and implement the funded supports in your NDIS Plan and link you to a range of community-based, independent and government services.

At Access Support Coordination, we help you to negotiate with providers about what they will offer you and how much it will cost out of your Plan. We ensure service agreements and service bookings are completed and that you have choice and control over your supports. We also prepare your end of plan report to inform your future funding.

Specialist Support Coordination

Specialist Support Coordination is funded if you have additional high or complex needs. This type of support will be provided by qualified Support Coordinators. Specialist Support Coordinators can also help you overcome barriers to accessing health, education, or justice services. The NDIS website has more information or ask one of our friendly staff.

Recovery Coach

The Recovery Coach role is designed to provide support to people with a psychosocial disability to increase their independence, social participation and economic participation.

A recovery coach will assist you to take more control of your life and to better manage complex challenges of day to day living. Through coaching, you will be supported to build capacity.

Recovery coaches will work collaboratively with you, your family and carers to design, plan, implement and review a recovery plan.

Supporting you to explore Supported Independent Living (SIL)

The team at Access Support Coordination are skilled in assisting eligible participants to explore Supported Independent Living (SIL) options.

SIL is a living arrangement that includes things like having a person to help with personal care tasks or cooking meals. SIL can help build these skills too, so you can be more independent. Our Support Coordinators can help you to achieve your independence through your goals and introducing you to professionals that are going to help you with the process all the way through.

Specialist Disability Accommodation (SDA)

We have strong connections with SDA providers within Queensland. Access Support Coordination can assist you in exploring SDA accommodation in your chosen areas.

There are no out-of-pocket expenses for Support Coordination. If you are eligible, the government pays for this support. We encourage you to discuss eligibility with our staff as we may be able to assist. You can also find more information on the NDIS Website, click on the link below.
